USUHS Department of Radiology Faculty Appointment and Re-Appointment Procedure

Thank you for your interest in joining our department. We are committed to excellence in USUHS medical student education, and, as such, expect our non-billeted faculty to be involved in teaching, research, or Departmental function. For radiologists requesting an appointment board certification is required, and for physicists and researchers, a PhD is required. The other requirements for appointment, re-appointment and/or promotion are outlined below.

Prefixes, Pathways, and Academic Ranks


Assistant Professor: New Appointment

Requirements:

1. Current CV.


2. Letter requesting USU appointment. Within the letter please specify the
     following:

        a. Why an appointment is desired
        b. Your qualifications and how you will enhance USUHS
        c. How your academic efforts will support or continue to support the             mission of the USUHS Department of Radiology

3. Completed USUHS_Form_107-1

Associate Professor and Professor:
New Appointments and Promotions

All appointments at the level of Associate Professor and above require an interview with the Chair of Radiology, and will be reviewed by an internal Promotions Committee.

Requirements:

1.  Current CV.


2.  Letter requesting USU appointment. Within the letter please specify the
     following:

        a. Why an appointment is desired
        b. Your qualifications and how you will enhance USUHS
        c. How your academic efforts will support or continue to support the            mission of the USUHS Department of Radiology

3.  Completed USUHS_Form_107-1


4.  3-5 names of reference: Actual solicitation of letters must be requested by
      the USUHS departmental chair and not directly by the candidate. No more
      than 2 local references should be from individuals at or above the
      requested academic rank.


5.  A synopsis of scholarly interests: A brief summary of your accomplishments
    and future plans vis a vis the USUHS categories of:

        a. Scholarship of Discovery (e.g. publications, book chapters,
            presentations, grants and funding, IRB protocols, etc)
        b. Scholarship of Teaching (e.g. medical students, residents, postgraduate,
            etc)
        c. Scholarship of Application (e.g. offices or committee memberships in
            National/International societies and organizations, Editorial Board
            memberships, etc.)
        d. Institutional Citizenship (e.g. activities in direct support of USUHS, SOM,
            Departmental and institutional committees and/or offices held)


6.  Reprints of your 5 most significant publications

Reappointment Process

All non-billeted academic appointments are reviewed every year. Although appointments are for 3 year cycles, we recognize that our faculty often changes status and/or employment more frequently. As such, all our academic appointments are evaluated annually. Reappointment is not automatic and requires documentation of continued activity in support of USUHS.

2. Completed Faculty Activity Form: Will be sent to the faculty member.
