2 month old male with infection of the left eye. Parents do not speak english.
Excessive tearing and palpable mass under skin medial to left eye
Cystic mass with mild peripheral enhancement
Dacrocystocele
Hemangioma
Dermoid cyst
Topical antibiotics and warm compress
Even though this looked like a 3rd eye - this patient was not so lucky and it turned out to be a Dacrocystocele.
A cystic expansion of the proximal segment of the nasolacrimal duct secondary to a congenital barrier. Dacrocystoceles, also called Timo cysts, can cause excessive tearing (epiphora) and infection.
Frequently recognized on prenatal ultrasound, Timo cysts usually spontaneously disappear within the first 8 months of life without complications.
The differential diagnosis includes anterior encephaloceles, hemangiomas and dermoid cysts.
