24 year-old woman with complaints of loss of visual acuity over the past 2 weeks.

Exam


Physical Exam and Laboratory

Physical exam reveals visual field changes involving the right eye.


Findings


Summary of Findings

Axial T2W MR image reveals enlargement of the optic nerve sheath complex on the right. Coronal contrast enhanced MR image shows enlargement and prominent enhancement of the right optic nerve sheath complex is present. Of note, the intracranial portion of the right optic nerve is not involved, and no abnormal areas of signal or enhancement were identified within the brain parenchyma.


Diffferential


Differential Diagnosis

Not applicable; However...
Consider optic neuritis as a manifestation of, most frequently, multiple sclerosis. Also consider optic neuritis as a manifestation of vasculitis. With involvement of the optic chiasm, consider, in addition to optic neuritis, chiasmal sarcoidosis and chiasmal neoplasms.
