History:
45 yo WF PMH sig for breast cancer w/ no post-mastectomy chemotherapy c/o Low Back Pain x 8 days, severely worsening over past 2 days. Pain onset upon awakening in lower thoracic and lumbar spines. Constant 10/10 pain, worse w/ twisting, a/w weakness in the lower extremities for past 2 days and radiation to left toes and right knee. Never had similar symptoms. Night prior to symptoms had been mopping at work. Seen at Bon Secours Hospital 7 days PTA. CXR and CT Scan performed revealing slipped/bulging disc and arthritis of vertebral column. No relief w/ Flexeril, Vioxx, and Percocet. No F/C, no N/V/D, no HA/photophobia/neck stiffness, no numbness/tingling, denies bladder/bowel symptoms, no saddle anesthesia, and no h/o trauma.
