DiagnosisC-spine fracture with locked facets
History47 year old man presents to neurosurgery clinic with a recent onset of neck pain and bilateral upper extremity paresthesias. Nine months previously He was seen in the ER, following MVA and then sent home.
FindingsCT following MVA in ER shows a subtle increased interspinous distance at C5-6 and minimally displaced inferior facet fracture on right at C6. Plain film done on the present admission shows locked facets at C5-C6 with marked anterior subluxation.
Differential DxHyperflexion injury with bilateral locked facets

DiagnosisC-spine fracture with left vertebral artery occlusion
History44 year old male in car crash. Multiple injuries to chest and neck.
FindingsFracture dislocation though the C2-C3 disc. Fractures involving the transverse formamena at C2. Occlusion of the vertebral artery at the C2 level with distal reconsitutionvia muscular collateral branches.

Findings• Fracture through the left transverse foramen, pedicle and lateral mass at C4. • Occlusion of left vertebral artery on CTA

DiagnosisC-spine fracture with spinal cord injury and vertebral artery dissection
History24 year old male who had too many beers prior to taking out his ATV. He drove the ATV into a tree and then tumbled down a 8 foot embankment.
FindingsCT-Scan C4 through C7 fracture dislocations MRI-Spinal cord edema and hemorrhage CTA- Traumatic occlusion proxiimal right vertebral artery
