DiagnosisMorgagni Hernia
History27 year old woman complaining of cough receives PA and lateral chest radiographs.
FindingsLarge right hemithorax mass seen on the PA view. Silhouette sign of the right heart border. Gas pockets within the superior portion of the mass. Lateral image demonstrates the mass to be anterior in the chest.

DiagnosisMorgagni Hernia (adult, incidental)
HistoryRLQ abdominal pain, CT ordered "rule out appendicitis"
FindingsTransverse colon lies above the level of the diaphragm, anterior to the heart and predominantly medial. No surrounding inflammatory change. Free passage of enteric contrast.
Differential DxThis is a classic location for a Morgagni's hernia, anterior and medial. This was small, no complications during fetal development (i.e Pulmonary hypoplasia). The main concern now is incarceration/strangulation, though there is no evidence complication on this CT.

DiagnosisMorgagni Hernia
HistoryNewborn child with trisomy 21. Cardiac echo revealed a ventricular septal defect.
FindingsNewborn films and those taken at 4 months old demonstrate bowel loops extending above the diaphragm anteriorly and to the left of the heart. The loops are more prominent on the 4 month radiographs. The heart is displaced to the right on the 4 month radiographs as well.

DiagnosisMorgagni Hernia
History55 yo male with 6 mo progressive shortness of breath. Quit smoking 13 mo ago.120 pack years.
FindingsCXR: The frontal projection demonstrates relics of previous pulmonary disease including plate-like atelectasis in the right upper and lower lung zones, and also in the left lower lung zones. There is a superimposed density on the right side which does not appear to represent the right diaphragm since this margin is visualized. The right hemidiaphragmatic mass is noted posteriorly, but not seen anteriorly, suggesting that the density noted anteriorly on the lateral projection and also overlying the right hemidiaphragmatic margin on the frontal projection probably represents anterior eventration of the right hemidiaphragm. Impression: 1. THERE IS THE SUGGESTION OF ANTERIOR EVENTRATION OF THE RIGHT HEMIDIAPHRAGM. MRI: Sagittal gradient echo T2-weighted images, demonstrate fatty mass adjacent to the heart with direct connection to the abdominal omentum. CT: Axial imaging demonstrates a fatty mass adjacent to the right border with multiple linear densities consistent with vessels.
Differential DxCardiophrenic Angle Masses Fat Pericardial Cyst Morgagni Hernia Lipoma Epicardial lymph nodes
