Generalization - the smaller the gland, the more malignant the lesion
PLEOMORPHIC ADENOMA (aka benign mixed tumor)
MONOMORPHIC ADENOMA - Warthin (cystadenoma lymphomatosum, adenolymphoma)), oncocytoma, basal cell adenoma, clear cell adenoma, sebaceous lymphadenoma
SOLID SOLITARY
"benign appearing" solid -
- child: LN, hemangioma, pleomorphic adenoma, lymphangioma, mucoepidermoid
- adult: pleomorphic, Warthin, mucoepi, adenoid cystic, acinic cell Ca
"malignant appearing": high grade mucoepi, adenoCa, undiff Ca
SOLID MULTIPLE
Warthin, acinic, lymphoma, granulomatous, metastasis
CYSTIC MULTIPLE
HIV (esp w/benign Cx adenopathy)
CYSTIC SOLITARY
branchial cleft cyst, Warthin, epithelial cyst
CALCIFICATION - pleomorphic adenoma, mucocpidermoid Ca, epithelial cyst, phlebotliths (hemangioma)
Tc pertech +> Warthin or ONCOCYTOMA
MRI LOW signal - high grade
Normal salivary glands:
PAROTID - largest (14-28gms)
begins at 4-6wks (10mm) from ectoderm
last to complete, capsule forms after lymphatics => intraparotid LN
SUBMANDIBULAR (10-15gms)
at 6wks (18mm)
SUBLINGUAL (2gms)
at 7-8wks (22mm)
MINOR GLANDS
at 12wks (62mm)
