The most prominent finding in this case is the enlarged symmetrical hilar and right paratracheal region c/w adenopathy. Slight fullness of the aortopulmonary window is also present. There are bilateral interstitial infiltrates radiating from the hila. In a young patient who is relatively asymptomatic, this combination of adenopathy and interstitial lung disease is most likely due to sarcoidosis. Differential diagnoses include infectious granulomatous disease such as TB and fungal infections, metastases, or lymphoma.
