labyrinthine lesion, recurrent attacks of veritgo w/tinnitus and deafness, last for minutes to hours, whirling/rotational vertigo, N&V tinnitus, fullness in ear are usually present, rotatory/horizontal nystagmus, the hearing loss usally precedes the first attack, but can come later, hearing decrements w/each attack, 90% unilateral, sensorineural hearing loss, M=F, 40's, dilatation (hydrops) of endolymphatic system => degeneration of hair cells, treatment is symptomatic (antihistamines, etc.) intentional destruction of labyrinth to relieve Sx

Newer Tx includes chemical ablation with intratympanic gentamicin therapy:
