Post-operative Biloma

In the first six images, there was progressive accumulation of radiotracer surrounding the right hepatic lobe and the dome of the liver that corresponds to the large collection seen on the CT scan of the abdomen. In the final six images, the radiotracer flowed through the drain (arrow) with a subsequent decrease in the activity surrounding the right hepatic lobe and the dome.

Hints & Teaching Points

Hepatobiliary scan obtained one day after the above CT scan. Dynamic images of the upper abdomen were obtained in the anterior view after intravenous injection of 7.6 mCi Tc-99m Mebrofenin for 60 minutes. During the first 30 minutes, the drain was clamped (images 1-6). After 30 minutes, the drain was then unclamped and imaging continued for another half an hour (images 7-12).
