DiagnosisCongenital Cholesteatoma
History4 year old male with presented with white mass behind the left tympanic membrane noted on routine physical exam. Pt has no hx of ear surgery, perforations or trauma.
Findings- Cholesteatoma of the left mesotympanum and epitympanum - Extends medially toward the malleus and incus without clear erosion of the bones - No dehiscence of the facial nerve noted

DiagnosisPRIMARY Cholesteatoma
History14 y/o male with intermittent otorrhea of right ear and right TM perforation.
FindingsSoft tissue opacity is present in the right middle ear within the epi- and mesotympanum, extending into the mastoid air cells. The right TM is perforated. The ossicles and tegmen are intact. However, the scutum is blunted.
Differential DxOtitis Media Cholesterol Granuloma Congenital Cholesteatoma

DiagnosisCholesteatoma
History22-year-old male complains of right ear fullness.
FindingsCoronal CT images of the right temporal bone demonstrate a soft tissue density localized to Prussak’s space with extension into the epitympanic recess. There is associated erosion of the scutum and thinning of the tegmen tympani. Abnormal soft tissue is also visualized within the hypotympanum.
Differential DxDifferential considerations for abnormal soft tissue within the middle ear cavity include Cholesteatoma, chronic otitis media related granulation tissue, paraganglioma, aberrant internal carotid artery, and dehiscent internal jugular vein.
