FindingsThere is a well circumscribed rounded lesion with increased density within the third ventricle incidentally noted on the non-contrast head CT. There is no associated ventriculomegaly. On MR, the lesion is hyperintense on T1 and relatively isointense on T2.

FindingsThere is a hyperintense spherical lesion at the superior aspect of the third ventricle on the FLAIR and T1 sequences, measuring approximately 5 mm in diameter. There is no enlargement of the ventricular system, suggesting that this lesion does not obstruct CSF flow through the third ventricle.

DiagnosisColloid cyst of the third ventricle
History27yo white man presents to the emergency room with acute onset of headache.
FindingsNoncontrast CT of the head demonstrates a well-circumscribed hyperdense mass in the anterior portion of third ventricle in the region of foramen of Monroe. There is asymmetric enlargement of the right ventricle suggestive of hydrocephalus. MR images demonstrate a well-circumscribed mass in the anterior portion of the third ventricle at the foramen of Monroe that is hyperintense on T1, mildly hyperintense on T2 and demonstrates some peripheral rim enhancement on post contrast images.

Findings11 x 11 x 13 mm nonenhancing well-circumscribed mildly T1 hyperintense and markedly T2 hypointense round mass in the anterior-superior third ventricle without ventricular enlargement.
Differential DxColloid cyst. Although other lesions can occur in this region such as ependymoma, subependymoma, giant cell astrocytoma and central neurocytoma,the imaging findings are characteristic for a colloid cyst.

DiagnosisColloid (neuroepithelial) cyst of the third ventricle.
History25 year old with prior bilateral ventricular shunting complained of poor short-term memory (Film .1 and .2).
FindingsSagittal noncontrast T1-weighted image (Film 1) reveals a homogeneously hyperintense oval unilocular mass of the anterior superior third ventricle at the foramen of Monro. The ventricles are decompressed, with left frontal and right parietal ventricular shunts in place. The axial T2-weighted image (Film 2) again demonstrates small ventricles after bilateral shunting. The mass is centered at the foramen of Monro and is homogeneous and markedly hypointense to both CSF and brain parenchyma. No other lesions are apparent.
