History52 y.o. man with difficulty walking, memory problems, incontinence; history of colon cancer
FindingsHead CT demonstrates hyperattenuating mass in the midline at the foramina of Monroe. The mass is causing obstructive hydrocephalus of the lateral ventricles with transependymal CSF migration.

HistoryA 25 year old female with headaches
FindingsThere is a well circumscribed rounded lesion with increased density within the third ventricle incidentally noted on the non-contrast head CT. There is no associated ventriculomegaly. On MR, the lesion is hyperintense on T1 and relatively isointense on T2.

History38 year old man with incidental finding on previous head CT.
FindingsThere is a hyperintense spherical lesion at the superior aspect of the third ventricle on the FLAIR and T1 sequences, measuring approximately 5 mm in diameter. There is no enlargement of the ventricular system, suggesting that this lesion does not obstruct CSF flow through the third ventricle.
Differential DxDifferential diagnosis includes: -Colloid cyst -Meningioma -Elongated Basilar Artery

DiagnosisColloid cyst of the third ventricle
History27yo white man presents to the emergency room with acute onset of headache.
FindingsNoncontrast CT of the head demonstrates a well-circumscribed hyperdense mass in the anterior portion of third ventricle in the region of foramen of Monroe. There is asymmetric enlargement of the right ventricle suggestive of hydrocephalus. MR images demonstrate a well-circumscribed mass in the anterior portion of the third ventricle at the foramen of Monroe that is hyperintense on T1, mildly hyperintense on T2 and demonstrates some peripheral rim enhancement on post contrast images.
Differential DxColloid Cyst Central Neurocytoma Gliomas (subependymal giant cell tumor in Tuberous Sclerosis, pilocytic astrocytoma, oligodendrogliomas) Craniopharyngioma Choroid Plexus Papilloma Vascular Tumors Cysticercosis
