Pituitary apoplexy is sudden infarction and/or hemorrhage of a pituitary adenoma. The apoplectic event can produce acute hypopituitarism and may cause blindiness by compresssion of the optic nerves and chiasm. The surgical specimens show varying proportions of hemorrhagic and ischemic necrosis. Immunostaining is usually not undertaken since it is not reliable and may give misleading results.

Apoplexy may also occur at birth or postpartum, when the physiologic enlargement of the pituitary gland undergoes abrupt hemodynamic deprivation causing necrosis and/or hemorrhage.
