Orbital pseudotumor, also known as idiopathic orbital inflammation
• most common cause of an intra-orbital mass in persons 10-40 years old.
• one of the most common causes of unilateral exophthalmos.
• commonly causes bilateral exophthalmos.
» Clinical triad:
• pain
• proptosis
• impaired ocular motility
» Two categories:
• acute: vasculitis with vessel wall necrosis and fibrinoid changes.
• chronic: inflammatory process with diffuse infiltration by lymphocytes, plasma cells, and macrophages.
» Associations:
• Wegener's midline granuloma
• fibrosing mediastinitis
• thyroiditis
• cholangitis
