Clinical Presentation: 61 y.o. man

History

61 y.o. man with bright red blood per recturm (hematochezia) and iron deficiency anemia.

Physical Examination and Laboratory

Gross blood in stool
Fe Deficiency

Image Findings

Solid column enema (gastrografin) shows fairly long segment smooth narrowing within the proximal to mid sigmoid.

Contrast enhanced CT shows circumferential wall thickening in the same location with associated infiltration of the subjacent mesenteric fat.

Differential Diagnosis

Colon cancer until proven otherwise.
Less likely lymphoma, metastatic disease, amebiasis, stricture.

Colon Cancer adenocarcinoma
Diagnosis Confirmed by: Surgery - Histology showed invasive moderately differentiated adenocarcinoma
