Follow-up pelvic ultrasound was requested for a 40 year old woman with an adnexal mass on a previous study.

Image Findings

FINDINGS
Left adnexal mass with homogeneous, low level internal echoes.
The lesion was unchanged in size or appearance when compared to a study that had been performed 3 years earlier.

Differential Diagnosis

DIFFERENTIAL DIAGNOSIS(complex cystic adnexal mass)
Endometrioma
Hemorrhagic ovarian cyst
Dermoid
Abscess
Cystadenocarcionoma
Ectopic pregnancy

Treatment and Followup

Tx and Followup:
The patient was under treatment for endometriosis; no specific treatment of the endometrioma was required.

Additional Discussion:
Although differentiation of complex cystic ovarian masses by ultrasound is difficult, this lesion demonstrates stability over 3 years and the characteristic ultrasound appearance of an endometrioma
