A serious and fulminating complication of cerebral abscess is rupture into the ventricular system. There is a tendency for abscesses to rupture medially into the ventricular system, because the medial wall is thinner than the lateral wall. This is related to the fact that white matter has less vascular supply than gray matter. A more intense vascular response is seen at the site of gray matter.
