9 month history of right eye proptosis and vertical diplopia. The patient denies eye pain, headache, or decrease in visual acuity. She denies weight loss, palpitations, skin changes, heat intolerance, or tremor. She denies any history of ocular trauma or sinus infections.
Proptosis, eyelid retraction, eyelid lag on downward gaze, and restriction with elevation and abduction.
Enlarged, enhancing right inferior rectus muscle. The insertion of the muscle onto the globe is spared. There is mild stranding of the surrounding orbital fat.
• Orbital myositis
• Thyroid ophthalmopathy
• Metastatic disease
• Orbital pseudotumor
• Lymphoma
