71-year-old Caucasian male with enlarging aortic arch aneurysm followed previously by serial CT examinations.
T1-weighted Spin Echo MR images ("black blood" technique) of the thoracic aorta at the level of the aortic arch demonstrates a 1.5-2.0 cm saccular aneurysm approximately 1.5 cm distal to the origin of the left subclavian artery. No dissection or rupture is identified. No additional vascular abnormalities are identified. Incidentally noted are bilateral nodular sub-pleural densities with a spiculated mass seen in the right posterior lung field.
Most thoracic aortic aneurysms are secondary to atherosclerosis (2).Additional etiologies include:
Connective tissue diseases (Marfan's and Ehler-Danlos)
Syphilis
Post-traumatic
Mycotic
Aortitis
Takayasu disease, Giant cell aortitis
Collagen vascular disease
Rheumatoid arthritis, Ankylosing spondylitis
