To date, no radiologic technique can reliably delineate the margins of diffuse (infiltrating) astrocytomas. Decades ago, in the late 1970's, it was recognized that microscopic tumor infiltration extended beyond the enhancing margins of the tumor. Later, we learned that neoplastic cells infiltrated into the region we had called "tumor related vasogenic edema".

The advent of MR gave new hope for discriminating between tumor, infiltration, edema, and normal brain. However, careful pathologic studies have shown that neoplastic cells are commonly found in brain regions with normal signal intensity on multple MR pulse sequences.

Attempts to use MR Spectroscopy (MRS) have also been largely unsuccessful.
