Additional Discussion:
This case demonstrates the usefulness of MR in diagnosing the following response to treatment in discitis and osteomyelitis. These entities will show decreased signal on T1WI due to increased water content. There will also be an increased T2 signal for the same reason. Post-gadolinium images are best for distinguishing the extent of the lesion and separating it from the inflammatory response. Gadolinium MR may also be helpful in differentiating an active acute inflammatory process from a chronic collection. MR is also useful for evaluating the extent of infection into the spinal canal and cord, particularly into the epidural space. However, it has been reported that in patients with concomitant epidural abscess and meningitis, CT myelography may be useful (3).
